What	Session-tier: the skill was never invoked in recent project sessions (when sessions exist).
Why	Never invoked is a fact. It is not a Remove candidate. A Recent install or a Global Skill stays here. See R034 for Authored Skills that are old enough to recommend remove.

How to fix

Confirm session adapters see this project (R030 if none found).
If it should fire: tighten the description (R005, R014, R025).
If it is Authored, old enough, and still never invoked: R034 and dora skill remove.
If it is intentional manual-only tooling, document that and expect low auto-invocation.
